The problem

Diabetes, hypertension and cardiovascular disease need continuous management. But across much of the continent, health systems are fragmented: a patient's data is scattered, care is episodic and reactive, practitioners lack a unified view, hospitals lose sight of people after discharge, and insurers price risk and pay claims on poor data.
Connectivity is intermittent and coverage is thin. The cost is preventable crises, avoidable readmissions, high claims and worse outcomes — for conditions that are, in principle, manageable.
